1. Open an image, select the part you want to use (I usually use the rectangular marquee tool holding down the shift key on the keyboard to have a perfect square), copy it (ctrl c), open a new document (ctrl n then click ok), paste it (ctrl v), resize it to 100x100px. This is your base.

2. Add a New Fill Layer: Layer –> New Fill Layer –> Solid Color… write ffeefa in the color palette; set this layer to mode: Soft Light;

3. Add a New Fill Layer: Layer –> New Fill Layer –> Solid Color… write c0eaff in the color palette; set this layer to mode: Overlay 33% opacity;

4. Duplicate your base (ctrl j), drag it on top of the overlay layer and set it to Soft Light;

5. Add a New Adjustement Layer: Layer –> New Adjustement Layer –> Selective Color… with these parameters:
Reds: Cyan: +81, Magenta: -21, Yellow: +19, Black: +14;
Neutrals: Cyan: +27, Magenta: +14, Yellow: -24, Black: +7;

6. Add a New Fill Layer: Layer –> New Fill Layer –> Solid Color… write 13243b in the color palette; set this layer to mode: Exclusion 20% opacity;

7. Add a New Adjustement Layer: Layer –> New Adjustement Layer –> Color Balance… with these parameters:
Midtones:: Color Levels: -32, -16, +14 with Preserve Luminosity checked;

8. Add a New Adjustement Layer: Layer –> New Adjustement Layer –> Levels… with these parameters:
Channel: RGB; Input Levels: 0 / 0.86 / 255; Output Levels: 0 / 255;

9. Add a New Fill Layer: Layer –> New Fill Layer –> Solid Color… write a3caff in the color palette; set this layer to mode: Soft Light 45% opacity;

10. Add some text, I wrote Sexy without trying (what I though of Ziva :P a long time ago) in… I don’t remember the font, color: ffffff; add a new layer and a border: select all (ctrl a), then click Edit from the menu bar and choose Stroke; in the new window use these parameters:
Stroke: Width: 3px, Color: ffffff;
Location: Center;
Blending: Mode: Normal, Opacity: 100%;

And you’re done!
