1. Open an image, select the part you want to use (I usually use the rectangular marquee tool holding down the shift key on the keyboard to have a perfect square), copy it (ctrl c), open a new document (ctrl n then click ok), paste it (ctrl v), resize it to 100x100px.

2. Duplicate your base three times (ctrl j) – you have 4 layers – change blanding mode of the three copies to:

3. Merge Visible (shift ctrl e); now this is your base.

4. Add a New Fill Layer: Layer –> New Fill Layer –> Solid Color… write 63f2ff in the color palette; set this layer to mode: Soft Light 30% opacity;

5. Add a New Adjustement Layer: Layer –> New Adjustement Layer –> Color Balance… with these parameters:
Midtones:: Color Levels: -20, 0, +20 with Preserve Luminosity checked;

6. Add a New Adjustement Layer: Layer –> New Adjustement Layer –> Selective Color… with these parameters:
Reds: Cyan: -30, Magenta: 0, Yellow: +30, Black: +20;
Yellows: Cyan: +100, Magenta: 0, Yellow: -100, Black: +13;
Cyans: Cyan: +100, Magenta: +49, Yellow: +30, Black: +18;
Whites: Cyan: +25, Magenta: 0, Yellow: +54, Black: -24;

7. Add a New Adjustement Layer: Layer –> New Adjustement Layer –> Hue/Saturation… with these parameters:
Reds:: Hue: 0, Saturation: -34, Lightness: 0;
Cyans:: Hue: 0, Saturation: -6, Lightness: 0;

8. Add a New Adjustement Layer: Layer –> New Adjustement Layer –> Selective Color… with these parameters:
Reds: Cyan: -30, Magenta: -11, Yellow: +2, Black: 0;
Yellows: Cyan: +20, Magenta: 0, Yellow: -30, Black: -5;
Magentas: Cyan: 0, Magenta: +14, Yellow: 0, Black: 0;
Whites: Cyan: -18, Magenta: -10, Yellow: +8, Black: -39;
Neutrals: Cyan: -13, Magenta: -1, Yellow: -20, Black: 0;
Blacks: Cyan: 0, Magenta: 0, Yellow: 0, Black: +30;

and you have this layer palette:

9. Go back to your base duplicate it three more times (ctrl j) – you have 4 layers plus the adjustement layers – change blanding mode of the three copies to (same as step 2):

10. Add a new layer and a border: select all (ctrl a), then click Edit from the menu bar and choose Stroke; in the new window use these parameters:
Stroke: Width: 3px, Color: ffffff;
Location: Center;
Blending: Mode: Normal, Opacity: 100%;

And you’re done! (grey border is only to let you see the white one when I have themes with white background :P )
